I will also provide a super bill for you if you intend to seek possible reimbursement from your insurance provider.

All you need to provide is a space for me to set up a mobile treatment table and a space to do some exercises. Being able to work with you in your home is an advantage to allow me to better understand what equipment and space you have to work with and integrate into your program. I will bring the table, KT tape, mobilization tools, cups, and bands if needed during our sessions. If you have further questions, I am happy to chat with you.
Not at this time. Accepted forms of payment are Cash, Credit, FSA or HSA prior to service. However, a “super bill” will be provided to you to help you claim any out of network health benefits your plan may offer. You can request reimbursement by submitting a claim to your insurance company. I recommend speaking with your insurance company before your session so they can answer any questions about the process.
No. You do not need a referral for PT services in the State of Oregon. However, I do recommend that you talk to your insurance company before hand if you are planning on submitting a claim for reimbursement. You may need a physician referral for this in some cases.
